Patient-Specific Simulation (PSS)
Просмотров 2457 лет назад
Current surgical simulations are generic with their inability to represent patient specific anatomic variations or the potential range of procedural complications. This video presents a novel preoperative simulation platform that marks a distinct shift in the use of simulation from a tool that allows practice of a specific skill (i.e., training) to a tool that allows rehearsal of a specific eve...

    eith , Putting away the resistance you get when moving the robotic arms, your movement towards the first target was smooth, you moved both arms together and got to the target in only 2 -3 movements At 0:12 you used your endowrist to grab the pellet (good) On moving towards the 2nd target you rotated the camera a bit but corrected it when you paused at the target On moving to target 3 I would have moved straight across then down to the target rather than take it in 3 moves like you did (0:33 to 0;37) Target 4 if you look at it again you will find that you moved in a zigzag motion zooming out then across then zooming in , moving straight across then zooming in would give you better economy of motion (0:43-0:47) Did very well with the target 5 , preventing rotation of the camera but you did hyperextend your arms too much at 0:57 (look at screen of your arms) ,that took you longer to recover (its good to use the extent of your arms but that was a bit more) Last target was very well done , I like how you corrected your camera at 1:15. 2nd go was better 2:18 try not to get too far out before zooming in to the target 2:30 same issue with 2nd target Might just be the faults in the simulator, I will make sure to correct that asap Summary Great coordination in moving the camera need to be be more efficient in your movements towards the target Great start keep up the good work

    Great performance Only comment at 1:20, 2:58 , 6:20!try not to switch from the right to the left arm stay with the right arm and engage your interest to twist the ring around the rail.

  • @aghazi01
    @aghazi01 7 лет назад

    Great performance Peter, I just have a single comment when you extract the needle from the pad don't force it. If you find that you are struggling and exerting excessive instrument force then let go of the needle and extract the needle using the other arm.

  • @aghazi01
    @aghazi01 7 лет назад

    Great work , when you are done with this task. I will need you to just complete 3 trails (no proficiency levels) on 2 exercises (tubes and Ring rail 2)

  • @aghazi01
    @aghazi01 7 лет назад

    that was a great performance 1- Needle exchange was seamless 2- You positioned the needle perfectly 3- Needle targeting was on point even with the hard targets at difficult angles Just need to move slightly faster.

  • @aghazi01
    @aghazi01 7 лет назад

    Great job with the approach of using the 4th arm and clutching when toggling between 4th and 1st arm few comments 1:28 minute: is realise that you moved the 1st arm without bringing it back into vision. 2:21 minute you were working with an outstretched arm (should have either engaged the clutch or moved the camera closer) this made it more difficult to move the ring along the tube. 2:32 minutes move the camera first leaving the arms in vision at all time, then follow with the arms that will streamline your movement better. 2:41 minute dont need to shift arms must keep using your right arm and engage the endowrist to maneuver that turn. did a lot better on the 2nd run

  • @aghazi01
    @aghazi01 7 лет назад

    Few comments 1- Pay more attention to your camera horizon, its preferred not to have the camera tilt beyond 45 degrees. 2- Try to move from one target to the other in the shortest distance. the guide will be the small blue arrow on the camera icon at 12 o'clock , follow its direction till you see the target then move directly to the target. if you are in the self-training group watch the tutorial for camera targeting again. its on this channel under DaVinci Simulator training videos playlist

  • @aghazi01
    @aghazi01 7 лет назад

    That was a great performance Peter the only comment I have was it 25 seconds where the fourth arm turned red. That demonstrates excessive instrument force . To avoid that you need to keep the fourth time in view of it while you are retracting the flap.

  • @aghazi01
    @aghazi01 7 лет назад

    Comments Try not to grab a traget (ring) with an outstretched hand) that will not enable you to be accurate with it (0:32 minute and 1 minute), move the camera closer You are moving the camera well, using the endowrist to twist the ring while placing it on the peg. Keep trying to refine your technique prior to moving a little faster.

  • @aghazi01
    @aghazi01 7 лет назад

    Comments 1- You tend to use one instrument at a time , try working both instruments together to complete the task. (1:24 minute)(6:16 minute) 2- Try cutting down this task (2:02-2:35 minute) into less steps , either move the 4ht arm with the camera and retract he flap from the very beginning or move the ring to as high as you can go without colliding with the 4th arm then move the 4th arm to retract the flap. 3- 3:55 take the ring as far as you can prior to engaging the 4th arm, that will speed things up Getting better keep practicing you need to think about this and plan ahead how you will carry out this task.

  • @aghazi01
    @aghazi01 7 лет назад

    Great performance You used the clutch in the first task, moved the camera in straight lines , keeping it at horizon most of the time (only time where the camera was > 45 degrees from horizon was from 1:01 to 1:07 minutes.

  • @aghazi01
    @aghazi01 7 лет назад

    Comments At 0:41 minutes,1:02 minutes, try to get closer to the target with the camera to prevent engaging with the target using your outstretched hand that decreases your accuracy in handling the ring. you did that in 1:29 minutes Good work with the camera movement, using the endowrist and moves directly to the target in a straight line.

  • @aghazi01
    @aghazi01 7 лет назад

    Hold the needle between the distal 2/3 and proximal 1/3 this will allow you to drive the needle across the whole target without the need to readjust (0:45 minutes, 2:29 minutes) Other than that your technique is good; you enter the target at the right angle and twist, you extract the needle with the contralateral arm. the only adjustment would be to hold the needle correctly so that you can take the needle through the targets in one swipe.

  • @aghazi01
    @aghazi01 7 лет назад

    At 0:39 minutes , 3:08 minutes you should have aimed you needle more acutely (ie tip towards the screen more that would line the tip of the needle with the target and also give you enough room to twist the needle. 1:43 minutes, you started to drive a needle with your wrist pronated that would hinder your ability to do it in a single throw when you switched to supination you could accomplish the throw. try to plan this ahead of time. 2:47 minutes, you are having issues with the backhand throws (forehand throws of the needle are fine). when throwing backhand targets start with your forearm in supination.

  • @aghazi01
    @aghazi01 7 лет назад

    I have only one comment, that was a perfect run. Keep up the good work.

  • @aghazi01
    @aghazi01 7 лет назад

    Overall great performance, few comments 0:28 minutes: try to never hold the needle parallel to the jaws of the needle driver you will not be able to direct the needle correctly Hold the needle between the distal 2/3 and proximal 1/3 at a 45-degree angle just like you did at 1:07 minutes (2:27 minutes is also a perfect technique)

  • @aghazi01
    @aghazi01 7 лет назад

    that was a great performance , you 1- Clutched while toggling between arms 2- Camera movement was synced with your arms 3- Used the endowrist when maneuvering the ring Great. With a little more practice you will get to the needed levels. FYI dont try to move faster just move more efficiently.

  • @aghazi01
    @aghazi01 7 лет назад

    Peter that was great , the only feedback was to be cognisant of your camera horizon usually it is preferred to be off by 90 degrees at max, that will make you move in shorter distances between the targets and prevent parts like 0:32 and 0:46 minutes.

  • @aghazi01
    @aghazi01 7 лет назад

    Good performance Positive comments 1- Clutch when switching between 4th and 1st arm 2- Camera movement was in sync Just one thing to take into account , at 0:56 and 1:30 minutes , always try to handle the ring with the instrument tips keeping them parallel (as you did at 1:07 minutes) that will allow easier rotation of the ring when twisting the master controls and prevent the snag that happened at 1:01 minutes.

  • @aghazi01
    @aghazi01 7 лет назад

    That was a great performance, FYI your collisions were between the needle driver and ends of the needle while turning the needle to position it.

  • @aghazi01
    @aghazi01 7 лет назад

    Great performance, You are handling the needle correctly, positioning it on the needle driver perfectly and using the endowrist to drive it. Only 2 things that I see needs improvement and that can save you time without sacrificing form 1- you take a few trials before the exit target turns green, try to estimate the depth of the needle prior to turning your wrist 2- Transitioning and positioning the needle from one task to the other takes a few turns for you , try to plan that as you are extracting the needle and position it for the next throw in the least number of needle flips (you did it best from 1:49 to 1:51 minutes)

  • @aghazi01
    @aghazi01 7 лет назад

    Positive comments 1- Using the 4th arm in the beginning but you should have taken it all the way to retracting the lip of tissue then moved back to take the ring all the way up to the first obstacle. 2- 1:15 minute you moved the camera closer when needed Changes 1- 1:03 minute need to be more aware of the camera not rotating that will throw you off. 2- You are having some issues with the endowrist it may sound weird but relax your grasp on the ring that will give you a wider range of moving the endowrist

  • @aghazi01
    @aghazi01 7 лет назад

    Again a great run Positive comments - Manipulating the needle at the pole during needle positioning - Using the curve of the needle when retrieving it - Aiming the tip of the needle using your endowrist - 2:50 is the most difficult target you executed that perfectly but should have let go with your left hand and use your right hand to extract it. The ideal way to hold the needle, at the junction between anterior 2/3 and posterior 1/3 at a 45 degrees angle. An example is 0:45 seconds (except you need to move the needle driver more anteriorly).

  • @aghazi01
    @aghazi01 7 лет назад

    Great run that eas excellent you utilized the endowrist adequately, managed the 4th arm without collisions , clutching when switching from one arm to another and moving the camera closer when needed to improve fine movements. The only thing that I would change (that actually did not get caught on your score) . if you look at using 0:27 sec when you retracted the tissue with the 4th arm you did so without moving the camera slightly upwards to visualize the arm which turned red (indicating excessive instrument force) , you will notice that better at 0:55 sec. Nevertheless, that was a great performance.

  • @aghazi01
    @aghazi01 7 лет назад

    Great run, I would just have you be aware of trying to keep your camera close to horizon (not > 90 degrees off). -Usually as you move from one target to the other if one arm moves above or below the other the camera will start rotating ( 0:20-0:22 sec) you either need to catch it early and prevent it, adjust prior on reaching the target (but that uses up your time) or adjust while moving to the next target (which you did at 0:46-0:49 but you rotated the opposite way).

  • @aghazi01
    @aghazi01 7 лет назад

    Good first run the previous exercise helped you a lot I have a few comments. You executed the first task perfectly you clutch to where you needed to in order to get to the target while your arms more comfortably in the console In the second target you started to rotate the camera back 24 seconds this usually happens when the left arm moves slower down in the right while you're moving across the screen. I don't know if you realize it but it seemed that you traveled a longer distance for that The third task was perfect , you followed the arrow on the crosshairs pointing towards the target , that small arrow moves around as you're moving towards the target your goal is to keep adjusting the camera so that you take the shortest distance between two targets. As for moving between the two targets that are on the side wall that was done very perfectly , where you were moving directly towards the target while at the same time rotating the camera. It almost seems effortless. Keep practicing you're doing very well

  • @aghazi01
    @aghazi01 7 лет назад

    Comments 1- Try to move both instruments together towards the target rather than keep on behind (0:30 min, 0:42 min) 2- Use the tip of the instruments to manipulate the ring that gives you better control on them (0:44, 0:57 min) 3- In the beginning its preferred to get close to the target that will save time when you pick the ring off the peg more precisely. (0:56 , 1:06 , 1:26 min) 4- 2:35 minutes you were to far from the target and thus you had to outstretch your arms, which lead to difficulty in manipulating the ring. Solve this by getting closer to the ring prior to removing it. You will understand this when you look at the difference in performance between 4:05-4:08 and 4:25-4:31 minutes. Concentrate on moving your instruments as one unit, get closer to the target and use the tips of your instruments to manipulate the ring. Keep practicing I need your time to below 50 sec.

  • @aghazi01
    @aghazi01 7 лет назад

    Comments - 0:24 to 0:36 minutes, great manipulation of the needle (using the poles of the needle to move it to position), great grasping of the needle with the driver (at the junction of 2/3 to 1/3 and slightly angled) and great throw and extraction. -2:09 to 2:20 this is a hard one , you still need to hold the needle 2/3 to 1/3 but must hold the needle from its inside then acutely angle the endowrist to point the tip of the needle towards the target. You almost got it at 5:14 minute. -5:34 , 6:34, 7:08 minute try not to hold the needle parallel to the needle driver. You will be very limited in your ability to rotate it. Great job for first few trials , you have the concepts down just try to hold the needle better and use the endowrist more to angle the needle.

  • @aghazi01
    @aghazi01 7 лет назад

    Great performance 1- Good endowrist manipulation of the ring along the tube. 2- Great strategy with coordinating the 4th arm and 2 other arms. 3- Adequately clutching on switching from right arm to the 4th arm and back comments 1:00 minute and at 1:30 min, try to get closer with the camera in this part that would have adjusted your arms to allow for finer manipulation and bought your forearms closer to the arm rest. Overall you may try to move slightly faster.

  • @aghazi01
    @aghazi01 7 лет назад

    Good work a few comments 1- Take care not to turn the camera (0:34 sec), as you move towards the target you will realize you were turning anti-clockwise by moving your left arm at a lower level than your right, keeping your forearms on the rest avoids that. 2- Try to keep your forearms on the arm rest (0:45 - 0:47 sec) 3- try to move to the target in a straight line t 1:01 sec should have moved directly towards the target 4- try not to lag your instruments behind your camera move them together this will avoid time needed to adjust the instruments to manipulate the target (1:12 to 1:14 sec). 5- 4:37 sec , 2 issues here, one your forearm needs to remain on the armrest, 2nd you need to activate the clutch function to manipulate this target which you did very well at 12:17 minutes. 6- 4:48 sec clutching and moving directly to the target would have made this part easier. 7- 6:44 to 7:10 minutes , 9:15 to 9:18 minutes you don't move efficiently in a straight line which may be due yo your camera being tilted , try to keep that at max to 45 degrees from horizon. 8- I would recommend either raising the arm rest or lowering your chair that will keep forearms on the arm rest and avoid tilting the camera offering you better economy of motion and thus a faster task completion time.
